Cool! Nobody ever asks me questions here so i’m psyched and you’re getting a detailed answer.. I’m not sure how much jailbreak experience you have so I’ll be detailed. 

1. Jailbreak your phone (If you haven’t already, I suggest going here http://greenpois0n.com/)

Now for the apps.  These are all available from Cydia. 

2. For the lockscreen date / time text you’ll need 3 things:

-Lockscreen Clock Hide

-Winterboard

-Typophone 4 (weather edition) 

3. For the icon swiping animation you need to get something called Barrel 

4. I have 5 icons in a row instead of 4. For that I have Five Icon Dock and Five column springboard.

And last but not least the animation that plays every time I lock my phone is called TV Tube Sleep it’s supposed to look like when you turn off an old tv.
